Step 1 - Weekly Practice #1

Please complete the pre-screening form and confirm your attendance in teampages for the session.

As reminder, this process, needs to be followed FOR EVERY SESSION that we have:

1. Confirm attendance in teampages (alerts are also sent out 24 hrs in advance)
2. Complete session pre-screening via Google Form

Once the form has been completed, you will get an email confirmation, please email me a copy of this
3. Complete session screening on site with me

At each session, players are to:
1. Arrive 15 mins early; dropped off at Oak Park
2. Please maintain a 2M distance
3. Do not touch cones, flags, nets etc
4. no throw ins, headers or touching the ball with your hands

Please complete on-site screening with Pam.
